Surrogate MotherThank you for considering becoming a surrogate mother with our medical practice. We appreciate your willingness to give prospective parents the gift of a lifetime through gestational surrogacy.

We want surrogate mother applicants to have a thorough understanding of the evaluation process so you know what to expect. CACRM has developed a unique physician-centered evaluation model where we can quickly assess your qualifications for the surrogacy journey. Our goal is to prepare you medically and emotionally for a healthy, safe, and satisfying experience.

Our surrogacy evaluation includes these components:

  • Meeting basic requirements
  • Completing an online application
  • Initial consultation with the CACRM surrogacy staff
  • Medical records review and background check
  • Psychologist evaluation and clearance
  • One-on-one discussion with the clinical team
  • Medical evaluation and clearance

Meeting Basic Requirements

Surrogate mothers should be between the ages of 21 and 40 and have had at least one successful, uncomplicated pregnancy and delivery, be in good health, not take any medications that could adversely affect the pregnancy and have a BMI (body mass index) under 30.

Completing an Online Application

Surrogate mother applicants first should complete our extensive online application where we will ask for detailed information on your:

  • Pregnancy and delivery history
  • Contraceptive use and menstrual record
  • Gynecological information
  • General medical and family history
  • Surrogacy experience

We also want to learn about your social and lifestyle habits and employment and insurance status.

The online application is an important evaluation tool that will let us know if you are ready to proceed to the next step.

Initial Consultation with CACRM Staff

After your online application is approved, you will meet with a member of our surrogacy team to learn more about what to expect as a gestational surrogate mother.

You also will complete a few additional forms and provide us with your medical records, including your complete obstetrical ones.

Medical Records Review and Background Check

We will carefully examine your medical records to ascertain any potential health or physical complications preventing you from becoming a surrogate mother. With your permission, we will conduct a criminal background check.

Psychologist Evaluation and Clearance

A psychologist who specializes in surrogacy evaluations will privately and confidentially interview you. The interview is your opportunity to ask any questions or share your concerns about surrogacy. The mental health specialist also will discuss the emotional and psychological effects of being a surrogate and its impact on your family.

One-on-One Discussion with the Surrogacy Team

Educating you about surrogacy is one of our top priorities. Our team will discuss the medical aspects of surrogacy with in vitro fertilization (IVF), including medication administration and guidelines for maintaining a healthy lifestyle during treatment and pregnancy.

Medical Evaluation and Clearance

You will undergo several tests before we approve you as medically ready for surrogacy. Dr. Arnold will conduct a physical exam, a hysteroscopy, and a pelvic ultrasound, and we will draw pre-conception blood work to make sure you are healthy for pregnancy.

A hysteroscopy is an endoscopic procedure where the doctor can inspect your uterine cavity for abnormalities. A pelvic ultrasound is a noninvasive diagnostic exam that produces images of the pelvic cavity. Both tests will give Dr. Arnold a thorough assessment of any uterine obstacles, such as fibroids or polyps, that might prevent a successful pregnancy.
